In the lifecycle of a trustworthy AI system, deployment is not the end of risk, it’s the beginning of accountability. By the time a system is live, it has often passed dozens of benchmarks, compliance checks, and test cases. But what happens next, after deployment, is when trust is truly tested.

Because sometimes, the model detects the problem, the logs record the anomaly, the dashboard flashes red, and yet, nothing happens.

This section explores the uncomfortable reality that detection is not the same as intervention. Modern AI systems may monitor their own outputs, performance metrics, and environmental signals, but these capabilities are useless if no one is positioned, or empowered, to act on them. What’s missing is not data. It’s responsibility.

It is governance without escalation, a system that observes, but cannot act.

To maintain trust in the operational stage, organizations must go beyond dashboards and logs. They must build escalation pathways, define authority roles, and ensure someone is explicitly tasked with saying “stop” when trust is at risk.
